Intensive Care Coordinator

The Intensive Care Coordinator (ICC) is responsible for overseeing the care coordination for youth clients in the foster care system or at risk of out of home placement. The ICC brings together all the providers and caregivers in a client’s life to assist the client in stabilizing their symptoms and improve functioning. This position leads the Child and Family Team meetings and provides direct therapy, skill building, case management, and parent sessions to the client and family.
